Landing in UK with Tier1 General visa and Medical Examinatio

Hi,
I have been issued a Tier1 General visa in may2010 valid for 2 years. It took me only 4 working days from the application to be submitted in Dubai UK consulate till my Tier1 General visa stamped from UK Embassy at AbuDhabi.

My question is; is there any time line I must have to enter the first time in UK with in some specified time or I can go there any time- the first time within 2 years?

Secondly, I have not been asked for any medical exam at all. Will I have to undergo medical exam after landing at the airport or may be in UK later on once I arrived there? Or do I need to undergo any medical exam from here Dubai (I line in Dubai) even before going for landing the first time and needs to carry medical exam reports with me? If yes then which medical tests, and what is the process?

It seems that I have to carry results for X-ray done from Dubai and to show at airport while entering the first time?
