How to apply VISA for baby born in UK, while I am on Tier-2

Hi
I am in UK on Tier-2 Visa and my baby is born here, I have got Indian passport for her. Now I need to visit India, for that I have to have dependent Visa for my baby. Unfortunately I couldn’t find any proper information in that direction from anywhere. Could you please guide me what all are documents do I need send (If I will have to send by post) i.e. Do I need to send all the passports(mine, my wife’s and baby) ? Do I need to send birth certificate as well ? I guess there has to be some form to apply VISA for new born baby.and To which address I need to send all the documents. I know that I need to send postal order worth £475 to Home office. Please advice me. Thanks in advance for any help.

Section2
C1. Under what nationality did the dependant enter the United Kingdom?

D1. Did the dependant obtain entry clearance or a visa before entering the United Kingdom?
(if No) Go to D5
D5. Please enter the reference number on the entry clearance or visa which can be found in the dependant’s passport or travel document.

D12. What is the dependant’s current immigration status in the United Kingdom?
