VAF 10 form clarifications

Hello,

I need some clarifcations on VAF10 form

1. My wife's first travelling was with her mother's passport in childhood. Does that count as 'her' passport ? Do I need to give details of that too..

2. Which address should be put in Q3.1 ? Passport or National ID one as they are different..

3. Old passports are required from the dependent. Do I need to include my old passport copies too ?

4. Can we can extra an extra page to fill out the details if not enough space present on the form.. e.g. for Q 8.1
