Visitor to Tier 2 (General)

i am switching from visitor status to tier 2 general. Do i need to go back home and apply?

My tier 2 General expired 3 months ago. i visited back home and now i am in UK on visitor visa. i had a job offer. I had one year experience as NHS doctor.
