Tourist Visa

Two years ago when I was having my tourist visa renewed (I've had it since I was kid; I'm now 29years old), the Embassy returned my passport and application with the was a notation "221 g" ( this was ticked off from a printed checklist) and instructions for me to come in anytime for an interview. I was given a year to accomplish this, unfortunately I did not prioritize this and did not appear.Now I'm in the process of renewing my tourist visa (again).

My question is, does my not appearing to come in for an interview affect me negatively now? Also, will that qualify as a denial even if I did not come in? There is a corresponding question regarding denials in the application form and I just want to make sure that my non appearance is not deemed as such.

In addition, my passport no longer has my "expired visa". Instead it has a stamp (where the visa was pasteds on) that says : CANCELLED WITHOUT PREJUDICE. In one of the questions of the application forms, it asks if my VISA has ever been cancelled or revoked. Is this a cancellation? Even if my visa expired already?

I hope someone can shed light to my query. In light of the above circumstances, by not appearing at the US Embassy for the requisite interview within the given year, is that deemed as a Denial? The question in the application form has stumped.

Any info or opinion will help. Thank you!
