Letter from Accountant for Selfemployed urgent,applying tomo

I have shown my earnings as self employed from freelancing work

What is the format for the letter from the Chartered accountant?

Also i will be applying on 17th december 2010

and the bank account statement for maintanence funds is dated 15th december 2010 and showing the transactions till 14th december 2010.

will this be ok or i have to get a bank statement on 16the december ?

requesting quick reply ,applying tomorrow.
