Human Case (Help needed please)

Dear All
-Please I have an enquiry about applying for a visa. Currently we are in the UK (me, my wife and children) as I have been doing a PhD since September 2006.
-Unfortunately my wife has been diagnosed with Leukaemia on Feb.2010, currently she is participating in a research study which will be for five years period from now. Our participation in the study is essential for us, she is getting a drug which has been approved in the USA but still not approved in the UK-NHS. This drug cost is about 1608 Sterling pounds per month and it is all supported by the Pharmaceutical companies not the British funds, public funds or any NHS support, but it is mandatory to take samples from my wife every six months in the UK for the period of five years, otherwise they will stop giving us the medication and certainly we will not be afford treatment. She is also not well especially with our young 4 and 8 years old.
-My enquiry now please how we can apply for extending my stay as my student visa will expire soon on January 2011. what is the category ? and can we apply for that visa while we are still in the UK? or we should apply back in home country? and is there anyway to get a long term visa to due to the health condition of my wife?
Please I hope to get a detailed answer and help as this issue is affecting my whole family life.

What's your wife's UK immigration status?

Are you eligible to continue studying.

Can you switch to PSW?

-My wife is on student dependent visa (as myself was studying).
-I can not continue studying after the PhD
- PSW= means post study work? I have to get my sponsoer approval who is the Egyptian government and I do not think they will allow that easily
