Payement by DD - Payer Name,Signiture ,Date

Hi All,

I want to make payement by Demand Draft.I have a query regarding "Under 28 Form, Section 3 - Payment Details".

Do i need to fill in Payer Name , Signiture and date(Date of the application or Date of Demand Draft)?

Please Help.
