HSMP first Extension to Tier 1 - dependants extension

Hi,

My friend is applying for his first extension from HSMP to Tier 1.

They have lost their marriage certificate, wife has husband's name on her passport, will it suffice as a proof of relationship ?
Is marriage certificat (nikah nama) mandatory ?
If they had submitted the marriage certificate with their initial application, do they need to submit the original certificate again with the extension application?
