Dividend Voucher Template

I will be applying for ILR in June-11. My current earnings are from a combination of employment and self employment. I am getting salary slips from my employment and have all P60s too. To meet the salary criteria, I would also need to include my earning from self employment which is a combination of salary+dividends.

1. Please can anyone provide me a dividend voucher template that they used previously? (I can pm you my email).
2. Do I also need to print out the salary (Director's fee) as well?
3. I am planning on providing the following as proof for earnings. Please can anyone confirm if this is sufficient? :
a) Salary slips + P60s + Personal bank statements (from employment)
b) Accountant's letter + dividend vouchers + Business bank statements + Personal bank statement (from self employment)[/url]
