Highly problematic. Generally, if you live in the U.S. (or just about any country) for less than 50% of a given year, you are jeopardizing green card & citizenship processes.
Given your circumstances, you have a good enough reason to be out of country for an extended period of time, but you better be in close contact with immigration and get very precise advise about it. Make sure you get documents and paperwork all in order and properly stamped & signed.
You might not need them .. but if you do in a year or so, the effort will be a small price.
Make sure you keep a permanent residency address in the U.S., even if you don't physically live at it while caring for your relative(s). Make sure you make a case that your presence is REQUIRED to help your mother ... being out of country OPTIONALLY will be far more problematic.
