EU 3 Permanent Residency ...Advice

EU 3 Permanent Residency ...Advice

Can anyone tell me if I will have any issues. Sorry, i have other posts as well but this is specifically to my circumstances.

1) Married to a EU national in 2006
2) My 5 years on EUfam is coming to an end next month
3) I am applying for a Permanent Residency.
4) I have been working in the last 7 years , never been out of work, thank God
5) EU spouse worked for about 3 and half years after wedding and 1 year before wedding.

6) My Wife had to leave the country for a medical treatment and had two operations in Czech Republic . She was away for about 11 months approx.

7) She was promised a job back on her return but things changed and she is now on jobseeker allowance and registered with FAS (also doing a course with FAS) right now.

8) We have been living in the same apt for 6 years
9) we both have VHI for the last 6 years paid by my me at first now my company pays


questions

I am filling out the form EU3. It is asking about my EU wife activity in the last 5 years but not mine??WHY??


In the form what box should I tick for her..in terms of her current activity.

Just to let you know, she had to get a p45 from her job before going to Czech in order to get her operations covered under Czech insurance.

Check the employment box and include a cover letter explaining the absence and enclose documentary evidence explaining the absence. Also state that she has been studying since and you have private medical insurance and include documentary evidence.

Also in the cover letter, state that you have been working for the last 7 years and include evidence of that.
