need help by filling passport form

I need help for filling passport form
section 3 as I ve 2 passport 1st one(expired) when I 1st entered in uk and 2nd one (also expired) on which I got ILR should I write 2 passport and then passport number and then send both passport with form as both or expired and on form it is written uncancelled form and there numbers or i should not write any passport number as they are expired
section 5 certificate of registeration or naturalization what i should write on place of issue home office london or london or council name??
can any one guide me regarding these please please

section 3
expired is not the same as cancelled (these are passports cancelled due to being lost, stolen).
Write "2" and the two passport number.
section 5
If I remember correctly "Home office London" should fit the field. If not, just "London".
