Basic details for British Passport

My Kid (1yr) has registered BC through MN1. Applying for British passport Now.

1. Can you please provide a link to download a link British passport aged 1year.

2. Is there any requirement in Application to have professional (GP/Health visitor) reference? if not i can save £50/-

3. We dont have permanent address in UK. I normally relocate depending on my next project.
a. Passport will be published with address?
b. If so, whenver I change address do I need to apply address change?

pick up form from post office, fill it up, professional have to sing it for you, and one of child pictures.
I don`t think you have inform them when you change address.

There is no address in the passport (most countries don't have address in the passport. I know that India do have an address in it). You will need to provide a UK address for the passport to be delivered to.
