CHANGE OF EMPLOYMENT APPLICATIONS

Currently I am in the UK with a Tier 2 CoS & my visa expires in Sept 2013. I am planing to change my employer but continue to do the same job at the new employer. There are no chnges to the salary or the job description other than the name of the employer.
My new employer is willing to get me a CoS as well.
Can you please answer following queries
1. What is a change of employment application?
2. How should it be submitted and by whom?
3. Do I still need to reapply for Visa?
I appreciate a prompt reply ASAP.

1. What is a change of employment application?

A (new) Tier 2 leave to remain application, on the basis of the (new) COS from the (new) employer.
