Lost passport visa transfer

Hi

My cousin lost his passport by SIA authority and he is fighting for his case.

He had Tier1 general (old rules) Visa since March - 2010.

In the mean time he got his new passport, and he wants to have his via re-issued on this new passport.

What form he should use for this and how much fee there would be?

I would appreciate your earliest response please.

If he's travelling, then he should apply for a TOC.
