cunfusion in psw application form

i am filling my psw form but i stuck on one small point and the point is:

In psw application form on page 34 in section o1 i tick no after that i have to go section to o5 and tick on page35 and cunfusion start here that if i tick then what letter i have to provide? if anyone could help me out , much appriciate. thankyou.

If you tick no it says to go to Section 5, not question O5.

i have to send my psw application on 26 nov 2011 and i got one point that:

My last statement date is started 1th november 2011 so just i want to know that is this acceptable because next statement will be generate 1 of dec 2011 and cannot wait till dec because visa expire in 30 nov 2011.

if someone can guide me ,much appriciate, thankyou
