EEA family permit

I am an Irish citizen living in the UK and want to apply for my Mother resident permit who is currently in the UK on visiting as she recently lost Her husband. How can I apply ? Also I have an under age step brother in Nigeria .Can I apply both for at the same time. My mum is 74 years in Uk and my step brother is 15 yrs old in Nigeria. Can someone pls tell me what forms and documents do I need.
