Some Silly; but Imp. Questions for Fillling up the HSMP1Form

Hi All forum members,

I have some question regarding filling up the new HSMP1 form.

1) Should one fill up this form on the MS-Word (Soft copy) or First take the print and then fill the form manually with his own handwriting?

2) Ticking a particular box. While checking any particular box, it comes as cross mark, which is reagarded as Tick Mark. Should I check mark in the soft copy or taking a print and tick mark by pen?

3) Fax Number: I do not have the Fax Number. Should I write as NOT AVAILABLE in the corresponding box?

4) Skipping the Irrelavant sections. Should I skip the irrelavant section as a blank or write NOT APPLICABLE in it?

all i can say is what i'm doing-

i'm filling all the boxes i can in electronically. then printing & signing.

the X is a tick- no need to retick

i'm leaving the parts that i don't need to fill in blank.
