company reference and Birth Certificate translation in eng

Hi All,

i have worked for 3 companies from
2000 to 2002 then
2002 to 2003 then
2003 to 2006 and now i am working in present company So do i need to give the details and contacts for every company .. or only the last company reference will do for me .....

and any one is having idea in bangalore where we can get the Birth Certificate translated in english and Notarised ...
