You should follow a bank letter or bank statement format as given in policy guidance para 99. Your and your team member name must appear on the bank letter or statement, if you cant do it then dont bother to apply as uk border agency can object on this.

I am also gonna apply as an entrepreneurial team and i had a chat with uk border agency on this issue and they said they not gonna accept anything other than their specified format for bank letter or statement as given in policy guidance para 99.
