Does the accountant nedd to issue accounts for my overall earnings or only self employed earnings?

However i would recommend to get both self employed and employed ...

Yes your accountant needed to issue accounts detail (Self employed) plus he should certify that your total gross income (Job + Self Employed) is (xyz)
