Doubts in form-fillup

HI, I have downloaded the hsmp application form(in pdf format).
In the first page(payment slip), section 2, no.7, Applicant's surname (please Print). Since it is in pdf document, its difficult to fill in Computer and print it. Any work around?

And in section 3, in credit card details, they ask 'valid from' and 'expiry date'. In 'valid from' they expect to write month and year. But in my credit card (visa), it only 'customer since : 2003), now month specified. So do I fill it.

Hi Indoswiss,

"Print" means write in capital letters. It does not mean Print as in take a computer print out. You can also download the PDF versions availabe on the Message board which you fill-up on your PC and take a print out.

As for your credit card, even if you have been a memeber since any year, rigth next to it, they would have writter, the validity period. There will be Valid from (month & year) and Valid till (month & year). It is an international practice and your card can not be different from the rest of us who also have a VISA card.
