Hi there,

Some answers below:

1. Can you please let me know if it is possible for my wife to change her status from Dependent visa holder to Work Permit holder?

Yes

2. Which form needs to be filled up by her Employer?
Probably the regular workpermit forms. There has been some change to the procedure since Apr 2004 I think.

3. Is it FLR-IED form with a work permit scheme?

Dunno about this.

4. Can this be done from UK only or she will have to go out of UK to get entry Clearance?

Yes, it will be an in-country application.
