I have got the HSMP approval letter and now have to apply for entry clearance (visa). I have a few questions, I hope ppl here can share some vauleable information:

i) The entry clearance form is Non-Settlement Form (VAF1 2003)? Please confirm?
ii) Section 5.1 of this form: How long do you intend to stay in UK?
What should be the answer to this question? In case of HSMP, the initial stay is around 1 year.
iii) Section 5.2: On what date do you intend to arrive?
If I specify some date for this question, do I need to provide them with ticket or proof of booking?
iv) Section 7: Employment in UK
Is it just for work-permit holders, or should HSMP holders also fill this secton.
v) How much bank balance is enough?

Do I need to attach anything else with the VISA application form other than
1.) HSMP Approval Letter
2.) 2 Photographs
3.) Bank Statement

Between 1000GBP to 1500GBP per month for principal applicant for a period of about 3 months.

500 GBP to 1000 GBP for each depedant for a period of about 3 months

Do I need to attach anything else with the VISA application form other than
1.) HSMP Approval Letter
2.) 2 Photographs
3.) Bank Statement

I plan to stay with a friend of mine. What kind of proof can I give for this arranged accomodation?
