ILR Eligibility and Dependent scenario SET(O) FLR(M) combine

Hi All,
I am on Tier 1 (Gen) and going to apply for ILR. My visa dates and holidays are as following:
Tier 1 General Visa Granted: 27 Jan 2009
First Entry in UK: 26 March 2009
Extended Once
Now my Tier 1 General Visa Valid Until: 6 Feb 2014

I do have total of following days spent out of UK in last five years:
1st Year: 87 days
2nd Year: 85 days + 7 days + 7 days
4th Year: 88 days

I would greatly appreciate if anyone can confirm following.
Q 1: Do I qualify for ILR?
Q 2: Confirm If the Form I need to use is Set(O)?
Q 3: My wife came as dependent in Aug 2012, so she is on Tier 1 Dependent visa and is not eligible for IRL for now. Does she need to use form FLR(M) to convert to Spouse Visa?
Q 3.b: Do we need one appointment at POE to apply both my SET(O) application and her FLR(M) application, or do we need two different appointments?

I would appreciate for any quick replies.
