please help AN form

hello every one i need help filling up form i booked to Submit my application next week with NCS i was looking at form today and confused about many things

!.1 previous border agency number

1.3 when given the indefinite. (i hold PR card)

2.1 first arrival in UK. i came on visit visa in 2004 later i went back home and applied spouse VISA and came in Feb 2007 not sure what date i need to enter.

2.4 and 2.5 do i need to fill up that section even i hold PR card as i am family member of EEA national.

3.11 details of police caution. i have police caution in may 2010 and can not remember the exact day. can i leave blank the day ?

section 5 referees i know many managers with VAT registered company is that ok 2 different managers working for same VAT registered company but from different branches can sign my application as a referees ?

can some one please help with above listed requirements

1.1- Leave blank

1.3- Your PR card should have a date and place of issue put that

2.1- Enter the day you first arrived with a view to staying in the UK on a long-term basis, this is the Day in Feb 2007 you landed enter the place as London if you landed in Heathrow or Edinburgh or Manchester or wherever you first landed.
2.4 and 2.5- YES you need to fill it up even if you hold PR Card.

3.11- Give month and year if you dont remember exact date and on page 15 fill out additional details of the case

Referees two different managers working for VAT reg company is fine, so long as the requirements are met.
