Tier 2 extension

Dear Memebers,

I have following question, can you please help.

My tier 2 extension is due end of july so can I apply three months ago for example end of april?
will my employer need to put date on COS 01-08-13 or 01-05-13?
Can you please let me know which application form i will need to use?
