Tier 2 General - Switching Employer

Dear All,
I am on Tier 2 - Sponsored Visa. this is my extended term which expires in June 2015. As per the calculation for PR Application, my 5 year finishes in Dec 2014. I am getting an offer from a new employer who is willing to apply my Visa. I have couple of queries, if someone can help or guide please:
1. How much transition period allowed for switching the employer?
2. What do i need from my new employer for sponsoring visa?
3. I have my family of Kid and wife do i need to change their visas too?
4. Importantly, will i be apply in Dec 2014 for the same PR application, even though i have changed my sponsor.

Can you suggest any good London based Immigration Advisor, whom I can personally meet and discuss the scenarios.

You need a CoS from the new sponsor which has to include RLMT and the job has to be NQF level 6 with appropriate salary (unless a shortage occupation or you are a high earner). Once you have the CoS you apply for a new leave but only give notice to your present employer after your application has been approved by the HO. Not sure what you mean by transition period.
