Stolen Passport with ILR Stamp

Is anyone able to advise me on the following:

1. Which form to use for reporting a stolen ILR stamp. I have been trying to call the Home Affairs Immigration Services for a couple of weeks but did not worry initially as I did not know I would need to travel and was also waiting for my passport.

2. I have a police crime number but cannot find the correct form online.

3. How to get a new stamp or no limit stay stamp if I do not have my passport to prove that I have kept up my entry requirements for ILR. I have the original letter. I got my ILR 12 years ago. I am getting my new passport on Monday and have been waiting for that before applying for ILR replacement stamp. Previous passport was electronic so probably somewhere the border agency has a record of my movements over the years.

4. I need to travel in the next few weeks due to close friend being diagnosed with cancer. I am happy to use emergency appointment but do not know which form.
