There are 2 type of Fiance' visa, on a simple fiance' visa applicant has to return to the country and re apply for Settlement visa.
Other type is Fiance' settlement visa and you have to pay for NHS surcharge and you can reapply in country for Settlement visa.

Fiance visa - No NHS surcharge due until applying for FLR(M) after the wedding.

Spouse settlement visa - NHS surcharge of £600 on application.
