Tier 1 G extension query

Tier 1 G extension query

Hello,

Could anyone please help me with below query.
I have done my Tier 1 G in 14 May 2010 which included dependent visa as well for my wife but we both separated in Aug 2010 and she headed back to India. Recently I have filled divorce case in England (March 2012) court and currently its in the process. so in marital status which option should i choose:
- married
- separated or separation order

if i choose separated then do i have to include copy of my divorce petition ?

my second query is:
My company reference letter has NJ ( new jersey) stamped but not UK as my HR team is based in India and NJ however they have mentioned my Uk address detail and UK company registration vat number etc... also in my all salary slips have got NJ stamp but not UK one do i have to concern about these?

My third query:
in the extension form page 11 B11, B12 & B14 asking for reference numbers what reference numbers should i put in?
B11 : Home office reference number if known
B12: Previous worker reference number if applicable
B14: Point based system migrant reference number if known

Q1 - I can not help unfortunately but I guess it will be Separated. However, wait until someone else comment
