Tier 1 General Extension Dividend Query

Hi

Please can someone help me with these 2 issues:

I am showing Salary as well as Dividend for my tier 1 hsmp extension. When calculating the dividend should I take net or gross dividend as some places on websites it shows net and some show gross. Please advice.

Also I am taking the dividend from my company's account into personal account so please advice should I submit my company’s bank account statement too along with my personal bank statement.
