ILR on May 9, final few questions

Folks, many thanks for all your help so far. I have prepared all the documents necessary for our ILR on May 9th. I have a few last questions:

Form Set O, question 5 . 3 Are you working in the UK? If so, what is your pay each month after income tax and other deductions
- I intend to mention the average of my annual net income as my monthly salary and dividends vary throughout the year. I suppose this field is not so important. Does anyone think otherwise?

Section 11L, For the period before you were granted leave as a Tier 1 (General) migrant, documents showing that you met the relevant requirements of the immigration rules
- My situation is - WP 2years + Tier2 1 Year + Tier1 2 Years. I am not sure what they will need as evidence for this case? as they have already seen my evidence while applying for Tier 1.

WP 2years + Tier2 1 Year was for same employer. Tier1 2 years have been as director of limited company.

Cohabitation documents - I am referring to Set M as this is not clearly mentioned in Set O.
If you did not live together for any part of the 2-year period, tell us the reasons for this and whether you stayed in contact with each other during this time, and provide any relevant supporting evidence.
My wife was on her own Tier2 visa and went to India for <3 months to resign and then switch in to Tier1 dependant. What will be acceptable evidence for this case? Do I need to show anything at all for <3 months period?
