Query for question J5 - Section 3 - Tier 1 Extn

Hi Gurus,

Query on question J5 in section 3 of the Tier 1 extension form where it says:

J5. The applicant should complete the following table giving details of previous earnings being claimed.

I have worked with two Employers in the past as a contractor.
I was hired by an employment agency as it happens for contractors and placed with at one of their client site. Then I would raise the time sheet at the end of the month, and the agency would transfer that money into my business account.
So, for the above question j3 should i mention the details about my Hiring Agency or about the Client where I was working or should I put details about my own limited company.

Query 2 - When doing the biometrics at PEO, do they also take the picture of the applicant apart from finger biometrics. Just curious to know.

Please advice Gurus..

Thanks for your reply. If I will put details about my own limited company and if they want to verify my earning who would they call then.

Contractors who have successfully got their extension can please shed some light on this. What details did you put on for this question.

You are employed by your own limited company as "director". The supporting evidence comes from your:

1. Payslips/dividend vouchers
2. Personal/business bank statements
3. Letter from your accountant confirming they are acting as your accountants.

For verification/employer contact: You should provided your accountant's contact details.

@WRS Clear insight but what if its a self employed business? Does the same thing apply as to how to fill the form and documents to submit

Even with the self employed business, don't you have the services of an accountant who is managing the book keeping?
