D. Immigration History

D1 – D3: Complete all questions. They refer to the most recent entry clearance that you entered the UK with.

D4:the first visa you had to enter the UK)

D5: Tick “yes” if you can not send your passport/s and complete

D6 to D10. Tick “no” if you are sending your passport/s and go straight to D11.

D6 – D10: You should answer these questions only if you cannot provide your current passport and/or any previous passports that you have used to travel to or stay in the UK. (D8: If you cannot remember all these details, provide as much information as you can and, if necessary, write “Not known” or “Cannot remember” under ticket booking or transport operator.)

D11: Current Visa Category ie Tier1

D12: If you are Tier1 tick no. go to D15

Tick “yes” if you are on a student visa
 D13- D14: Sponsorship by a government or international scholarship agency means “wholly supported by an award covering both fees and living costs.” If you are sponsored by a government or by an international scholarship agency, but your sponsorship does not pay for all your tuition fees and living costs, you should answer “No” to this question and you do not need to provide a letter from your sponsor. The same applies if you are sponsored in any way by a person or an organisation that is not a government or international scholarship agency.


D15: This is the date of expiry of your current visa.

D16-D17: Tick “yes” and complete D17 if you have overstayed your visa. Otherwise tick “no”.

D18-D19: Tick “yes” and complete D19 if you have worked more than your visa allows. Otherwise tick “no”

D20-D21: Tick “yes” if you have ever come to the UK illegally and complete D21. Otherwise tick “no”.

D22 – D23: Tick “yes” if you have used deception, for example forged documents, to get a visa and complete D23. Otherwise tick “no”.

D24-D25: Tick “yes” if you have been removed from the UK and complete D25. Otherwise tick “no”.


D26-D29: Tick “yes” if you have any other applications with the Home Office and complete D27 to D29. Otherwise tick “no”.

 D30-D32: Tick “yes” if you currently have an appeal to be heard and complete D31
