Yes

You will get same points as that you claimed in your initial application, as points granted are the same as that claimed during initial application

Unfortunately, the current fee structure shows dependent application when applied with main applicant has a higher fees than when applied seperately. So, no discount

2) Should I follow the option of ll other (Applications for leave to remain)
ALL OTHER APPLICATIONS FOR LEAVE TO REMAIN
* PHD 45 points
* Masters degree 35 points
* Bachelor's degree 30 points
Similarly in H4 of page 33, should I tick the box:
* Original certificate of award
or
* Evidence previously provided with the applicant's initial Tier 1 (general) application

if you provided letter from UKNARIC in your initial application then select 'Qualification not on the points based calculator, but a UK NARIC letter and/or certificate was provided with the
applicant’s initial Tier 1 (General) application'

No, select the first of choices, see below for H3
H3. Please indicate the points being claimed for the applicant’s qualification including equivalent professional/vocational qualifications:
Applications for leave to remain where the applicant has, or has had, leave as a Highly Skilled Migrant, as a Writer, Composer or Artist, Self-employed Lawyer, or as a Tier 1 (General) Migrant under the Rules in place before 6 April 2010, and has not been granted leave in any categories other than these since 6 April 2010
PhD 50 points
Masters degree 35 points
Bachelors degree* 30 points
Similarly in H4 of page 33, should I tick the box:

Yes. The points elaborated directly under "Applications for leave to remain where the applicant has, or has had, leave as a Highly Skilled Migrant, as a Writer, Composer or Artist, Self-employed Lawyer, or as a Tier 1 (General) Migrant under the Rules in place before 6 April 2010, and has not been granted leave in any categories other than these since 6 April 2010" on page 37 is what you should use to determine the number of points you can claim. As you say your earnings will be £28000 then the box you have mentioned above would be correct.
2) Similarly, question for J4, page 35 where I need to mention the Start and End date of earnings?
I'm not sure what the question is here, but what you should state here is the date range (12 consecutive months within the 15 prior to your application) for which you are claiming points for.
3) Would you recommend enclosing the two pages from UKBA (when they successfully granted me the visa by issuing that letter) case worker where they mentioned the points given on UKBA letter head? Just to make case worker life easier, as you mentioned
You can do so, but this is not mandatory. I personally didn't provide this, and applied under the same rules as you, earlier this year without a problem.
