Choosing ILR dependent form

I am a tier 4 dependent , i will complete my ILR In July 2012 (5yrs students+3yrs PBS dependent+ 2yrs Tier 4 dependent) my question is to apply ILR for my wife( Tier 4 student we got married over 4yrs) which form does she need to use SET(M) or FLR(M) and does she need Life in the Uk test or ESOL A1 test? Please kindly answer my questions.
