Documents for baby visa from Pakistan

Hi,

I am planning to apply for my daughter visa and visa centre in pakistan have requested the following documents along with application

Sponsor letter (from the main applicant so i am the main applicant)
Accomodation letter
bank statement

Can anybody please provide format for sponsor and accomodation letter.

also if i attach bank statement for my wife accounts does she have to provide sponsor letter as well.

Please also share your experience if you have applied from back home.
