How earliest Tier-2 (ICT) VISA extension can be applied ?

I have following questions regarding Tier 2 (ICT-Established Staff) visa extension:

1) How earliest I can make an application for extension? my visa expiry is on 22 December 2012, I have plans to travel during this period.
2) If I make an extension application at least 3 months before the expiry date and decision comes within next one month then would I loose 2 months from my current VISA or extension will be applicable 22nd December 2012 onwards?
