Urgent Help - SET(O) Section 5 (applying on 12th June)

Dear Experts,
Im applying for ILR using SET-O in PEO Solihull on the 12th June. Im currently in the UK on Teir 2 General. While reviewing my filled in form I realized the last part of Section 5 says - "You must complete ALL 4 of the boxes below... "
1. SOC Code =
2. Current Rate of Pay (as stated in COP - hourly rate or annual salary) --> __per hour
3. Current Rate of Pay (as stated in COP - hourly rate or annual salary)--> __per Annum
4. Number of hours this salary is based on =
5. Number of weekly hours =


My employer has already given me the letter in the format given in this forum stating my SOC code (2132) and stating that my annual salary is 40k which is above the stated salary for SOC 2132 - 31200 per annum. What Im not so sure about the 5 boxes above is :
1. Do I only have to fill in one of the 2 boxes from box 2 and Box 3 i.e. would it be sufficent to state the per annum salary as thats the only figure available in COP pdf or do I need to work out hourly rate as well based on this per annum figure
2. In box 3 the figure should be the one given in COP against SOC 2132 which 31200 or should this be the salary I AM receiving i.e 40,000 per annum
2. for point 3 which is the "Number of hours this salary is based on " could I simply populate a figure arrived at by multiplying 8 hour/day into the number of working days in a year = 2080 approx , I cant find any details in the Tier 2 Occupations COP version11/11

These questions might sound inane but Im a nervous wreck at the moment and want to be absolutely sure that Im not making any mistakes in the form. Appreciate your guidance.
