Human rights article 8 based on zambrano

Can someone help me please. I want to make an application based on the human rights articles 8 (zambrano)and I have to do it myself as I can't afford a lawyer . Please can someone give a head start with what I will need to submit ?

Hi all u need to do is to download the eea2 application form from UKBA website, file those pages that are applicable to u, u will see list of other items u need to include and the 3 most important evidence they need to see which are ur dependants passport as evidence that he or she is British, evidence of ur relationship which is birth certificate, evidence to show that the child is totally depends on u for his welfare, these could be letters from school, GP, Church and nursery etc, as long as it is addressed to u as the carer or parent of the child. I did mine myself and I was issued a COA, but if u did not include a passport they will not process the application.

Is comprehensive sickness insurance required for this type of application?
Also do you complete the retained rights of residence section?
Do you need to support the application with bank statements?
