Tier 1 gen extension query

Hi all
I am applying for Tier 1 general extension. Need your help for few of my queries


Date initial tier1 gen application submitted: 7 july2010 (under rules before 19july 2010)

Date requested for UK visa in application form to enter UK: 1 Sep 2010

Date on which application was successful and got the passport with the visa stamped: 11 july2010

Date from which visa given: 1 Sep 2010

Date entered UK: 1 Oct 2010

Date of expiry of current leave: 1Sep 2012


Query1:
Question in the section for immigration history – ‘what date was the entry clearance or visa is issued?

Should I give the visa start date (1 sep 2010) or application date (7 july 2010)?

Query2:
Does booking appointment for premium service, include biometric as well or a separate appointment is required for that

Query3:
If I don’t get appointment and apply via post how early should I send the application? if I send by 1st aug will it be ok ?

Query4:
Should biometric procedure be also completed before current visa end date?

Query5:
Home office sends documents by secure delivery. Should I be present to collect it or my friend in my home can collect on behalf of me?

Does this question ask "on what date did you make an application on the basis of which entry clearance or visa was issued"?

Regarding query1, ques doesn't mention about the application date

It just says -- D3. What date was the entry clearance or visa issued?
