ILR Dependant form

Hi,

My husband is planning to apply for ILR (Tier one + work permit) this June 2012 and I was wondering if I, as his dependant, and our little boy need a separate form from his SET(O) to apply for ILR.
I couldn't find any form in the home office website for this purpose. Can we (his dependants) just be included in his SET(O) form?
