Moving Tier 2(skilled worker) to Tier 1(General) Dependant

I am on Tier 1(General) and my current leave expiring on 4th March 2013, residing in UK since Feb 2005. My fiancee is on Tier 2( General and on NHS sponsorship visa) and her leave expiring on 5th March 2013. She is living in Uk since march 2010. We are living together for more than 2 years, but not married or registered , neither have any joint accounts.
When I am applying for Tier 1 Extension in Feb 2013, will it be possible for us to apply together. ie, making her application as Tier 1(G) dependant. What all requirements we have to meet before applying. She need to exit country?
Am sure this might have been asked here before, but I could not find any threads with such information.
Any advise/directions will be greatly appreciated.

If she is applying as dependant she need to apply from the home country.
