discretionary leave to remain

hi i got the discretionary leave to remain for 3 years and its finishing on 20th september 2012 i did not apply any asylum i got it based on life in uk my wife and 2 kids are british when i first time appllied, i fill the form first time was flr(m) but this time i am confused which form i should apply it is the same or its new one showing one on the website is hpdl form when i write on the website for the extention form for discretionary leave its coming hpdl form. please if anyone can tell me exactly thanks
