PBS Defendant version 06/2012 and What Fee(s) To Pay

Q1. I was previously on a WP and now trying to apply for Tier 2 General from WP.

I had filled the PBS Defendant version 06/2012 which is effective from 14/06/2012 for my 4 dependents to be attach with my application (me being the main applicant). However, when I submitted this to my employer and she said she thinks the form is wrong.

Not sure if I have have got the wrong form or if she is unfamiliar with it because it's a new form. Can someone please direct me to where I can get the correct form if I have got it wrong.


Q2. On page 6 of the form (PBS Defendant application Form)
I am also a bit unsure as to the Fee Details I should be making for myself and each dependent.

Any help of feedback would be appreciated.
