tier 1 general dependant extension

hello,
I have few doubts in filling the dependant application. I am currently in the UK.

1.Question no. H1. what is the Home office or UK border agency number. Where can i get that info from? from my visa?

2. Question no. L3. Reference number of the main applicant's most recent application?
The main applicant is submitting the papers along with the dependant. what would be the reference number and date then?

3. What would be the application fee for child under age 3?

2. Leave it blank if you don't know.
3. Same fee as adult dependent.

I've left that blank on my initial dependent application (successful) and on an extension application (currently under consideration). I've also helped a friend fill up the same form, left it blank and she didn't have any problem getting the visa.

In short, it hasn't been a problem until now.
