Bringing baby to UK from India

Hi,

Many thanks for the very informative forum. Much appreciated.

Background:

My second daughter was born in March 2012 in India. Both me, my wife and my elder daughter have ILR since June 2011. In November 2012, I plan to bring all of them in UK. I have not yet applied for her Indian passport in Mumbai

Question:
1) Which form should I fill to bring my second daughter in UK? VAF4A? or any other form for Entry Clearance?
2) My elder daughter's passport expires in March 2013 and we plan to travel back to UK in Nov 2012. Will this be an issue?
3) Which paper appendix should I use for my daughter's VAF4A application?
According to UKBA site, both paper Appendix 1 and Appendix 2 state this criterion -
The child of settled parents or with one parent who has limited leave as a partner or parent granted under the Rules in force before 9 July 2012.

I received my ILR in June 2011

However, on the online application site (visa4u.co.uk), Appendix 1 does not state the above criteria. Its only Appendix 2

Any guidance much appreciated as always.
