British Passport for Child

Hi,

I just want to know if some one got details of the following,

I have a child born in UK and we have just received her Nationatility Certificate MN(1) approval and now we need to apply for a passport and therefore just wondering what documents should be submit. As we both parents are on ILR and child just recieved the approval thereforeshould we provide the following documents,

1. Old Pakistani Passport (for Child)
2. Child's Birth Certificate
3. Our Pakistani (English Translation) Nikkah Form
4. MN1 Approval letter

Is there anything else should we need to sent such as our passport, ILR Biometric card, the HO letter etc? Please confirm if you have been to the same condition.
